TEAM ADMINISTRATION FORM
In an effort to ensure that all members of the COWHA are up to date on the affairs
of their teams the executive has asked that all coaches ensure that their managers
have this form completed as directed. Should parents have any questions regarding the
administration of their respective teams they should contact their team manager or head coach.
Any unresolved issue may be directed in writing to the executive committee.

All team managers will provide parents with a copy of this form. All parents must agree to the release of information in respect to their child prior to team information being released, posted or printed.
